Notice of Privacy Practices

Effective Date: August 11, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This notice applies to Titan T Center and its workforce. Titan T Center is a self-pay health care practice. We do not submit claims to insurance, but we accept eligible HSA and FSA payments and may disclose information for payment or at your request when permitted by law.

Your Rights

You have the right to:

  • Get an electronic or paper copy of your medical record.
  • Ask us to correct your medical record.
  • Request confidential communications.
  • Ask us to limit what we use or share.
  • Get a list of certain disclosures of your information.
  • Get a paper or electronic copy of this notice.
  • Choose someone to act for you.
  • File a complaint if you believe your privacy rights have been violated.

Get an Electronic or Paper Copy of Your Medical Record

You may ask to see or receive an electronic or paper copy of your medical record and other health information we maintain about you. Contact us to learn how to submit your request. We will usually provide a copy or summary within 30 days. We may charge a reasonable, cost-based fee as permitted by law.

Ask Us to Correct Your Medical Record

You may ask us to correct health information that you believe is incorrect or incomplete. We may deny the request in some circumstances, but we will explain the reason in writing, generally within 60 days.

Request Confidential Communications

You may ask us to contact you in a specific way, such as only at a particular telephone number, or to send mail to a different address. We will agree to reasonable requests.

Ask Us to Limit What We Use or Share

You may ask us not to use or share certain health information for treatment, payment, or health care operations. We are generally not required to agree. If we agree, we may still disclose the information if needed for emergency treatment.

If you pay for a health care item or service out of pocket in full, you may ask us not to disclose information about that item or service to a health plan for payment or health care operations. We will agree unless the law requires the disclosure.

Get a List of Certain Disclosures

You may ask for an accounting of certain disclosures we made during the six years before your request. The accounting will not include disclosures for treatment, payment, or health care operations and certain other disclosures excluded by law. We will provide one accounting in a 12-month period without charge. We may charge a reasonable, cost-based fee for another request during the same 12-month period.

Get a Copy of This Notice

You may ask for a paper copy of this notice at any time, even if you agreed to receive it electronically. We will provide it promptly.

Choose Someone to Act for You

If a person has legal authority to act for you, such as through a valid health care power of attorney or guardianship, that person may exercise your rights and make choices about your health information. We will verify the person’s authority before acting.

File a Complaint

You may complain to Titan T Center if you believe your privacy rights have been violated. Contact our Privacy Officer using the information at the end of this notice.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by mailing a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting hhs.gov/hipaa/filing-a-complaint.

We will not retaliate against you for filing a complaint.

Your Choices

For certain health information, you may tell us your choices about what we share. Tell us your preference, and we will follow your instructions when the law gives you that choice.

You may tell us whether to:

  • Share information with family, close friends, or others involved in your care or payment for your care.
  • Share information in a disaster-relief situation.

If you cannot tell us your preference, such as when you are unconscious, we may share information if we believe it is in your best interest. We may also share information when needed to lessen a serious and imminent threat to health or safety.

We will obtain your written authorization before using or disclosing PHI for marketing when HIPAA requires authorization, before selling PHI, and before most uses or disclosures of psychotherapy notes. We may contact you about treatment alternatives, health-related services, appointment reminders, treatment follow-ups, or refill information as permitted by law.

We may contact you for fundraising efforts, but you may tell us not to contact you again. If we maintain substance use disorder patient records protected by 42 CFR Part 2, we will provide clear advance notice and a choice before using Part 2 information for fundraising communications.

How We Typically Use and Share Your Health Information

Treat You

We may use your health information and share it with professionals involved in your care. This may include laboratories, pharmacies, referring providers, consultants, and other treating professionals.

Example: We send a laboratory order and relevant clinical information to an outside laboratory, or we send a prescription to your pharmacy.

Run Our Organization

We may use and disclose your health information to operate our practice, coordinate services, assess care, train staff, manage records, conduct quality and compliance activities, and contact you when necessary. Business associates that handle PHI for us must protect it as required by law and contract.

Example: We use health information to manage your treatment plan, schedule follow-up care, and review the quality of our services.

Receive Payment

We may use and disclose health information to collect payment for services and to support HSA, FSA, financing, or payment transactions. Although Titan does not bill insurance, we may provide records or receipts at your request or disclose information when otherwise permitted or required by law.

Example: Our payment processor receives information needed to complete your payment.

Other Uses and Disclosures

We may use or disclose health information in other ways permitted or required by law. We must meet the conditions in the law before making these disclosures.

Public Health and Safety

We may disclose health information for public health and safety activities, such as preventing or controlling disease, reporting adverse events or product problems, helping with recalls, reporting suspected abuse or neglect, and preventing or reducing a serious threat to health or safety.

Health Oversight

We may disclose health information to health oversight agencies for activities authorized by law, including audits, investigations, inspections, licensure, and disciplinary matters.

Research

We may use or disclose health information for research only when the requirements of applicable law are met.

Comply With the Law

We will disclose information when state or federal law requires it. This includes disclosures to the U.S. Department of Health and Human Services when it requests information to determine our compliance with federal privacy law.

Organ and Tissue Donation

We may disclose health information to organ procurement organizations when applicable.

Medical Examiners and Funeral Directors

We may disclose health information to a coroner, medical examiner, or funeral director when a person dies and the disclosure is permitted by law.

Workers’ Compensation, Law Enforcement, and Government Requests

We may use or disclose health information for workers’ compensation claims, certain law-enforcement purposes, health oversight activities, and special government functions, when permitted or required by law.

We may disclose health information in response to a valid court or administrative order, subpoena, discovery request, or other lawful process, after satisfying applicable legal requirements.

Authorized Record Requests

We may send your records to another provider or person when you sign a valid authorization or when another legal basis permits the disclosure. Titan does not represent that it participates in a formal health information exchange solely because it responds to authorized record requests.

Substance Use Disorder Records

To the extent we maintain substance use disorder patient records protected by 42 CFR Part 2, we will not use or disclose information from those records in a civil, criminal, administrative, or legislative investigation or proceeding against you without your written consent or a court order and subpoena that meet applicable requirements.

Part 2 records received under a general consent for treatment, payment, or health care operations may be used and redisclosed as permitted by HIPAA, except that they may not be used or disclosed in proceedings against you without your written consent or a qualifying court order and subpoena.

Our Responsibilities

  • We are required by law to maintain the privacy and security of your protected health information.
  • We will notify you as required by law if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in the notice currently in effect and provide you with a copy upon request.
  • We will not use or disclose your information other than as described in this notice unless you authorize us in writing. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
  • When Tennessee or another applicable law provides greater privacy protection than HIPAA, we will follow the more protective law.

Changes to This Notice

We may change this notice and make the revised notice effective for health information we already maintain and information we receive in the future. The current notice will be available upon request, at our office, and on our website. The notice will show its effective date.
